Sleep Is the Keystone: What the Evidence Actually Shows

Among all the interventions in preventative medicine, sleep is the one with the most unforgiving evidence. Restrict healthy young adults to five hours a night for one week and you can measure the damage: insulin sensitivity falls by roughly 25%, testosterone drops by 10–15%, evening cortisol rises, and blood pressure regulation deteriorates.

These are not associations from questionnaires; they are controlled-laboratory findings, reproduced repeatedly. Sleep is not recovery from life; it is the physiological process the rest of your health is built on.

The two questions that matter

Clinically, sleep evaluation begins with two questions: are you getting enough, and is the sleep you get intact? The first is behavioral and honest tracking usually answers it. The second is medical, obstructive sleep apnea affects an estimated one in four middle-aged men, most undiagnosed, and it quietly drives hypertension, atrial fibrillation, insulin resistance, and low testosterone.

If you snore, wake unrefreshed despite adequate hours, or carry treatment-resistant hypertension, a home sleep study is one of the highest-yield tests in medicine. It is simple, inexpensive, and frequently life-changing.

The most effective sleep interventions remain unglamorous: a consistent wake time, morning light exposure, caffeine discipline after noon, alcohol honesty, and a cool, dark room.
