Our commitment
Nader Medical is required by law to maintain the privacy and security of your protected health information (PHI), to provide you this notice of our legal duties and privacy practices, and to notify you in the event of a breach of unsecured PHI.
How we may use your information
We use and disclose PHI for treatment (coordinating your care, ordering labs, prescribing), payment (processing membership and lab fees), and healthcare operations (quality improvement, compliance). Other uses and disclosures require your written authorization, which you may revoke at any time.
Your rights
You have the right to inspect and obtain a copy of your health records, request amendments, request an accounting of disclosures, request restrictions on certain uses, request confidential communications, and receive a paper copy of this notice.
Security measures
All telemedicine visits, messaging, and record storage occur on HIPAA-compliant, encrypted platforms under Business Associate Agreements. Access to your PHI is limited to personnel involved in your care.
Questions and complaints
If you believe your privacy rights have been violated, you may file a complaint with the practice at naderbran@gmail.com or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
